Employees were performing roofing approximately 13 feet above ground level without the use of a fall protection system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a) Jobsite at or near 4318-4320 Walnut Street, Philadelphia, PA - On or about August 27, 2025, the employer did not ensure that the ladder used to access the upper landing surface of the roof was extended to at least 3 feet. Employees used ladders that were not extended at least 3 feet to access the upper landing surface of the roof to gain access to the roof, thereby exposing employees to fall hazards approximately to 13 feet.
