29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: a) Jobsite located at 6191 Ridge Ave, Philadelphia, PA. - The employer did not ensure that an employee was provided a safe means to access and egress the scaffold. Employee had to access and egress the scaffold by climbing on the scaffold frames and cross braces; thus exposing employee to fall hazards. Condition observed on or about August 27, 2025.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system. a) Jobsite: 6191 Ridge Ave, Philadelphia, PA - The employer did not ensure employees performing siding and roofing activities were protected from falls by guardrail systems, safety net system, or personal fall arrest system. The employees were working from the roof of a 2 story building without the use of fall protection, thereby exposing employees to fall hazards. Condition observed on or about August 27, 2025.
