29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) 425 Ben Franklin Boulevard, Lancaster, PA 17601: On or about August 20, 2025, employees were exposed to burn injuries from a release of bone broth from Kettle #29 and were not provided with and/or required to wear personal protective equipment (PPE) to prevent burns. b) 425 Ben Franklin Boulevard, Lancaster, PA 17601: On or about August 28, 2025, an employee was exposed to burn injuries from a release of bone broth after stacked bins on a hand pallet truck struck a control panel to a kettle and the employee was not provided with and/or required to wear personal protective equipment (PPE) to prevent burns. No abatement certification required.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: a) 425 Ben Franklin Boulevard, Lancaster, PA 17601: On or about August 20, 2025, employees were exposed to severe burn injuries from a release of bone broth from Kettle #29 and were not provided with or required to wear personal protective equipment (PPE) which would have prevented burns. The employer did not prepare a written workplace PPE hazard assessment that outlined the hazards present which necessitated the use of the PPE. b) 425 Ben Franklin Boulevard, Lancaster, PA 17601: On or about August 28, 2025, an employee was exposed to severe burn injuries from a release of bone broth after stacked bins on a hand pallet truck struck a control panel to a kettle and the employee was not provided with or required to wear personal protective equipment (PPE) which would have prevented burns. The employer did not prepare a written workplace PPE hazard assessment that outlined the hazards present which necessitated the use of the PPE. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
