29 CFR  1910.132(d)(1)(i):The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  On or about August 21, 2025, in the production area of the facility, employees did not have the adequate PPE while moving metal bars from a bandsaw loading tray.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On or about August 21, 2025, in the production area of the facility,  an employee was working on a bandsaw and the tip of the employee's right thumb was caught between metal bars and the housing of the bandsaw. The employee was taken to the hospital where a section of the right thumb tip was amputated. The employer did not report the incident to OSHA within 24 hours.
