29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On or about August 21, 2025, in the production area of the facility, an employee was working on a bandsaw and the tip of the employee's right thumb was caught between metal bars and the housing of the bandsaw. The employee was taken to the hospital where a section of the right thumb tip was amputated. The employer did not report the incident to OSHA within 24 hours.
