29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact. a) Shipping Merge - On or about August 24, 2025, employees performing servicing and maintenance on Bed No. 5702.06 were exposed to hazardous energy and moving machine parts during machine restart. Before lockout or tagout devices were removed and energy was restored, the employer did not ensure that an authorized employee inspected the work area to confirm that guards were reinstalled, resulting in the amputation of the right-hand index fingertip.
