29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: Buckeye Basement Solutions at the jobsite located at 12941 Township Road 220 in Rushsylvania, Ohio; On or about July 16, 2025, the employer did not ensure employees were protected from coming into contact with electrical power circuits through the course of work. Employees engaged in the repair and replacement of subfloor and joists of a residential home were exposed to electrical hazards associated with contact with 220 volts in that the electrical energy feeding the house wiring was not isolated.

29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take: Buckeye Basement Solutions at the jobsite located at 12941 Township Road 220 in Rushsylvania, Ohio; On or about July 16, 2025, the employer did not ensure through direct observation or by instrumentation whether any part of an energized electrical power circuit was located in a manner to which a person, tool, or machine could come into physical or electrical contact. Employees engaged in the repair and replacement of subfloor and joists of a residential home were exposed to electrical shock hazards associated with contact with 220 volts in that the electrical energy feeding the house wiring was not isolated.

Employees engaged in the repair and replacement of subfloor and joists of a residential home were exposed to electrical hazards associated with contact with 220 volts in that the electrical energy feeding the house wiring was not isolated. Instance B On or On or about July 16, 2025, the employer did not ensure each employee was trained in the recognition and avoidance of unsafe workplace conditions, and the applicable regulations to control or eliminate hazards. Employees were exposed to shock hazards associated with contact with 220 volts in that employees without adequate electrical knowledge engaged in electrical work such as, but not limited to, cutting and terminating 220 volts electrical wiring into junction boxes.
