29 CFR  1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  (a) Boric Acid Room: On August 25, 2025, point of operation guards were not permanently affixed on the Asset #1435 Manu Stokes Tablet machine asset.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate (a) Boric Acid Room: Between July 15, 2025 through July 31, 2025, the employer did not ensure employees who serviced equipment were trained as "authorized" employees. Employees performing die setting tasks on tablet press machines such as but not limited on the Asset#1435 Manu Stokes Tablet machine were not provided with the knowledge and skills needed for the safe application, usage, and removal of energy control.  (b) Boric Acid Room: On August 25, 2025, the employer did not ensure employees who serviced equipment were trained as "authorized" employees. Employees performing cleaning tasks to dies during operation such as but not limited on the Asset#1435 Manu Stokes Tablet machine were not provided with the knowledge and skills needed for the safe application, usage, and removal of energy control.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (b) Boric Acid Room: Between July 15, 2025 through July 31, 2025, employees were exposed to machine hazards associated with moving parts when conducting changing out dies on tablet presses such as but not limited to Asset#1435 Manu Stokes Tablet machine. The employer did not implement energy control application steps such as but not limited to lockout tagout when performing changing out dies [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  (a) Boric Acid Room:  On August 25, 2025, employees were exposed to machine hazards associated with moving parts when cleaning dies while in operation on tablet press machines such as but not limited to the  Asset#1435 Manu Stokes Tablet machine. The employer did not implement energy control application steps such as lockout tagout when cleaning dies during operation [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  (a) Boric Acid Room: On August 28, 2025, the employer failed to conduct periodic inspections of each energy control procedure at least annually to verify the procedures were adequate and were being applied properly.  This would include an inspection of the machine specific procedures for the Asset#1435 Manu Stokes Tablet machine.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
