(a) Worksite: On or about August 28, 2025, employees were exposed to amputation hazards while utilizing a portable table saw that was not guarded.

(b) Worksite: On or about August 28, 2025, employees were exposed to electrical hazards when a circular saw was plugged into a multi-outlet power strip, with multiple devices plugged in.
