On or about August 28th, 2025 Location: 27 Sandy Brook Dr, Spring Valley, NY 10977  a) Employees were exposed to approximately 10 ft fall while accessing the top of the wood frame with an A-frame ladder. Employees were not trained in the use of a correct ladder.
