29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about August 28, 2025, the employer exposed employees to crushing and struck-by hazards in that employees operated the Kubota Wheel Loader (Model R640, Serial KBC5Z63CANZD10724) with a custom universal quick attach trailer mover attached without obtaining the forklift manufacturer's written approval. This trailer mover is attached to the quick coupler of the forklift and is used to move Recreational Vehicles (RV), to and from the parking lot.

29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On or about August 28, 2025, employees operated the Kubota Wheel Loader (Model R640, Serial KBC5Z63CANZD10724) with a custom universal quick attach trailer mover attached, to move Recreational Vehicles (RV) in and out of the parking lot. The forklift was not appropriately marked to identify the new load data when the trailer mover was utilized, thus exposing employees to struck-by and crushing hazards.
