Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) In the maintenance area, located at 4701 95th St. N, Saint Petersburg, FL 33708: On or about 08/28/2025, the employer exposed employees operating a Nissan PF02A20V forklift to crush-by and tip over hazards in that, the forks were modified by adding a tow hitch onto the forks without receiving approval from the manufacturer or from a licensed engineer. b) In the maintenance area, located at 4701 95th St. N, Saint Petersburg, FL 33708: On or about 08/28/2025, the employer exposed employees operating a Crown C51050-50 forklift to crush-by and tip over hazards in that, the forks were modified by adding a tow hitch onto the forks without receiving approval from the manufacturer or from a licensed engineer
