29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  (a) Under-construction residential structure, west side garage roof; On or about August 27, 2025, a worker was not protected from falls while riding on a steel platform attachment placed on the forks of a Pettibone brand model 8044 all-terrain forklift to access and egress the roof at an approximate height of 11-feet above ground-level.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the capacity or safe operation of the equipment were made without the manufacturer's written approval, and/or if such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:   (a) Under-construction residential structure, west side garage roof; On or about August 27, 2025, a fabricated steel platform placed on the forks of a Pettibone brand model 8044 all-terrain forklift was used to transport a worker to and from elevated locations and neither the forklift nor platform were identified with capacity ratings, limitations, etc. and/or the attachment was not approved by the manufacturer for such use.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Under-construction residential structure, west side garage roof; On or about August 27, 2025, a worker was not protected from falls while engaged in framing construction activities from a 12:12 pitch roof where the lower eave edge was approximately 11-feet above ground-level.
