Employees operating at approximately 18.3 feet or more above a lower level and were not protected from falls while engaged in residential construction activities. Employees were installing fascia and soffit at a new construction multi-story duplex.

Employees were utilizing ladder jack scaffold that was tied to together with straps and resting on a awning. Employees were utilizing the ladder jack scaffold while installing fascia and soffit at a new construction multi-story duplex.
