Employees were not retrained on the unique fall hazards associated with the designed unguarded opening on the platform decking which exposed employees to falls in excess of seven feet.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not report the hospitalization of employee to OSHA within the required twenty-four hours.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
