29 CFR 1926.503(c)(1) The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill when changes in the workplace render previous training obsolete: a) 32355 Wallops Island Rd Chincoteague: On or about August 14, 2025 and times prior, employees at the jobsite worked on the top level of a rocket launch platform finalizing the piping for the deluge system. Employees were not retrained on the unique fall hazards associated with the designed unguarded opening on the platform decking which exposed employees to falls in excess of seven feet. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) 32355 Wallops Island Rd Chincoteague: On August 13, 2025, an employee fell six feet from edge of concrete onto piping below resulting in injuries that required inpatient hospitalization. The employer did not report the hospitalization of employee to OSHA within the required twenty-four hours. b) 32355 Wallops Island Rd Chincoteague: On August 14, 2025, an employee fell through a floor opening and received injuries that required inpatient hospitalization. The employer did not report the hospitalization of employee to OSHA within the required twenty-four hours. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
