29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) Lauro Navarro DBA Lauro Navarro failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected.  Citation 1, Item 1 a serious violation of 29 CFR 1926.102(a)(1) with an abatement date of February 4, 2026.  Citation 1, Item 2(a) a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of February 4, 2026.  Citation 1, Item 2(b) a serious violation of 29 CFR 1926.502(d)(7) with an abatement date of February 4, 2026.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about August 27, 2025, at the jobsite located at 3211 N. 15th Street, Milwaukee, WI, the employer did not ensure employees performing roofing work on a residential home with pneumatic nail guns were utilizing safety glasses, exposing employees to eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) On or about August 27, 2025, at the jobsite located at 3211 N. 15th Street, Milwaukee, WI, employees were working on a two-story residential home without any fall protection in place, exposing employees to a fall hazard of approximately 15 feet to the ground below.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   (a) On or about August 27, 2025, at the jobsite located at 3211 N. 15th Street, Milwaukee WI, the employer had not ensured the attachment point of the body harness was located in the center of the wearer's back by clipping it to the D-Ring on the harness, while performing roofing work on an existing residential home. This exposed employees to a approximately 15 feet fall.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  (a) On or about August 27, 2025 at the jobsite located at 3211 N. 15th Street, Milwaukee, WI, the employer did not ensure the extension ladder used by employees to access and egress the south side of the residential roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 15 feet to the ground below.  (b)  On or about August 27, 2025 at the jobsite located at 3211 N. 15th Street, Milwaukee, WI, the employer did not ensure the extension ladder used by employees to access and egress the north  side of the residential roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 15 feet to the ground below.
