29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. a) For the entrance to the packaging area from the loading dock, on or about 09/02/2025 and the preceding two months, the employer exposed employees to struck-by hazards from forklift tip over, in that, floor holes ranging from 2.5 to 7 inches in diameter as well as a floor hole that was 6 inches by 12 inches were present in an entranceway used by forklifts that could come into contact with the floor holes.
