29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) 211 South New Holland Road, Kinzers, PA - On or about June 6, 2025, employees performed servicing and maintenance activities on such equipment as but not limited to CNC machine, Universal 700 planer, pin router,  table saws. The employer did not develop and implement an energy control program, consisting of machine-specific energy control procedures, employee training,  and periodic audits exposing employees  caught-in and struck-by hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition.  a) 211 South New Holland Road, Kinzers, PA - On or about June 6, 2025, The TCM - Model FHG 30 N5T sit-down rider forklift, was not removed from service when found to be defective, including but not limed to a missing seat belt buckle.  No abatement certification or documentation required.

29 CFR  1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of fixed dual router station:  a) 211 South New Holland Road, Kinzers, PA - Building A - The employer did not ensure the two Makita portable routers (RD1101 and RF1101) were adequately guarded at the point of operation to prevent employees from inadvertently contacting the rotating router bit, exposing employees to  stuck-by hazards, on or about June 6, 2025.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) 211 South New Holland Road, Kinzers, PA - Building A -  The employer did not visually inspect the two Makita portable routers (RD1101 and  RF1101) before use as evidenced by cord where the outer jacket had separated near router's body end.  Employees had this cord plugged into the two-outlet receptacle box, exposing employees to electrical and fire hazards, on or about June 6, 2025.  No abatement certification or documentation required.
