29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  a)Job site, 4738 Clairton Boulevard, Pittsburgh, PA: On or about September 2, 2025 during siding activities, the employer did not ensure the scaffold where employees worked was properly set up. Deficiencies noted during the inspection included, but were not limited to, not using fall protection on the scaffold, the scaffold platform was not wide enough and the platform was too far from the face of the work.

Deficiencies noted during the inspection included, but were not limited to, not using fall protection on the scaffold, the scaffold platform was not wide enough and the platform was too far from the face of the work.

29 CFR  1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent person designated by the employer:   a)Job site, 4738 Clairton Boulevard, Pittsburgh, PA: On or about September 2, 2025 during siding activities, the employer did not ensure a system of  hazard identification and correction was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, not having the electrical service drop blanketed, not using appropriate personal protective equipment such as hardhats, not using fall protection on a scaffold, scaffold platform not being wide enough and the scaffold platform being too far from the face of the work.
