29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  383 Southampton Road, Westfield, MA: On or about September 2, 2025, an employee was observed working on a roof with unprotected sides and edges greater than 6 feet to the next lower level and were not protected by guardrails, a safety net system or personal fall arrest system.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:   383 Southampton Road, Westfield, MA: On or about September 2, 2025, an employee was exposed to fall hazards as part of his assigned duties and the employer had not trained him prior to exposure to these hazards in topics such as but not limited to recognition of fall hazards, dangers of fall hazards and procedures each employee must take to prevent or minimize risk of such exposures.
