29 CFR  1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  (a)Riverside Industries L.L.C. at 40361 200th Street, Huron, SD 57350:  On or about and at times prior to August 14, 2025, the employer did not ensure I-beams stored in tiers were stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. Subsequently, an employee working near a stack of I-beams that were being moved by a forklift operator was struck by a falling beam from the stack causing a fracture to the employee's knee.  This condition exposed employees to struck-by and crushing hazards.

This condition exposed employees to struck-by hazards associated with improper forklift operation.  (d)Riverside Industries L.L.C. at 40361 200th Street, Huron, SD 57350:  On or about and at times prior to September 2, 2025, the employer did not ensure that the Toyota 50 Forklift #4, Model Number: 8FBCHU25, Serial Number: 64538 powered industrial truck was inspected at least daily.
