29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: a. On or about September 3, 2025, at the above-mentioned workplace, employees operating the Exeter Model 2206R fleshing machine on an elevated platform were exposed to fall hazards of seven (7) feet to concrete walking working surface due to the absence of a top guard rail, safety net system or personal fall protective system located directly behind the machine operator.
