29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) On or about September 2, 2025, the controlling employer failed to ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the jobsite, materials, and equipment; conducted by a competent person. Subcontractor employees were conducting residential roofing activities without utilizing a fall protection system. This condition exposed subcontractor employees to a fall hazard of approximately 20 feet.   b) On or about September 3, 2025, employees of Martin Quintanilla Luna, the subcontractor, were exposed to fall hazards of approximately 10 feet to the ground below while conducting roofing activities on an existing roof without utilizing fall protection.  As the controlling employer, RMConstruction, Inc., failed to ensure that frequent and regular safety inspections of the jobsite located at 7524 E. 121st Place, Thornton, CO were conducted to ensure subcontractor employees were protected from falls.
