Commercial Street Smyrna, DE 19977(side yard): On or about September 3, 2025, employees were electrically shocked while conducting construction activities contacting a 14.4 kW energized electrical line that was not guarded.

Commercial Street Smyrna, DE 19977(side yard): On or about September 3, 2025, employees were exposed to fall hazards when the employer did not ensure that employees conducting gutter replacement work and were exposed to falls of approximately 10 feet were trained in the recognition and mitigation of such hazards to recognize the hazards associated and how to fall hazards at the site.
