29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. At an establishment located at 8791 Frank Drive, Minocqua, Wisconsin, in the Old Showroom, employees were exposed to electric and fire hazards from exposed conductors in the following instances; a. On or about September 3, 2025, a three-gang light switch box on the western wall of the old showroom did not have a faceplate. b. On or about September 3, 2025, a junction box on the western wall of the old showroom did not have a cover. c. On or about September 3, 2025, a junction box along the southern wall of the Old Showroom did not have a cover. d. On or about September 3, 2025, a single-gang receptacle box along the southeastern wall of the Old Showroom did not have a faceplate. e. On or about September 3, 2025, a single-gang receptacle box along the eastern wall of the Old Showroom did not have a faceplate. f. On or about September 3, 2025, a single-gang receptacle box along the northeastern wall of the Old Showroom did not have a faceplate. g. On or about September 3, 2025, a junction box along the northern wall of the Old Showroom did not have a cover. h. On or about September 3, 2025, a double-gang light switch box on the western wall of the old showroom did not have a faceplate.

29 CFR 1910.178(l)(6):The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): On or about September 3, 2025, at an establishment located at 8791 Frank Drive, Minocqua, Wisconsin, the employer did not certify that employees operating the Hyster S50FT LP forklift had been trained and evaluated as required. All employees had prior experience operating forklifts, but the employer could not provide documentation that the employees had been trained on workplace required topics.
