29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). On September 3rd, 2025, employees engaged in residential construction activities located 8800 South 33rd Street, Lincoln, Nebraska 68516, were exposed to fall hazards in that employees were observed working from an aerial device without any form of fall protection. The General Contractor failed to conduct frequent and regular inspections of the sub contractors performing work on their job site.

29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about September 3rd, 2025, employees engaged in construction activities located at 8800 South 33rd, Street, Lincoln, Nebraska 68516, were exposed to fall hazards in that employees were observed working from an aerial device without any from of fall protection. Instances of failure include but are not limited to: 1.) Electrical sub-contractor employees working from aerial device without fall protection on September 3rd, 2025. 2.) Siding sub-contractor employees working from aerial device without fall protection on September 5th, 2025.
