29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about September 3rd, 2025, employees engaged in electrical construction operations located at 8800 South 33rd Street, Lincoln, Nebraska 68516, were exposed to struck-by hazards in that employees were observed working on the ground directly below an aerial lift that had guys working above them. The employees working on the ground were not wearing protective helmets.

29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:(a) On or about September 3rd, 2025, employees engaged in electrical construction operations located 8800 South 33rd Street, Lincoln, Nebraska 68516, were exposed to fall hazards in that multiple employees were observed working from an aerial device and they were not wearing or utilizing any form of fall protection. In addition, the fall protection that the employer did provide, did not properly fit the employees that were required to wear it.
