29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is not greater than one inch. On or about September 3, 2025, it was observed that employees engaged in masonry activities on the fitness building of a 425 multi-family apartment complex located at 8800 South 33rd Street, Lincoln, Nebraska were exposed to fall hazards in that the employer failed to ensure that each working level of a scaffolding was fully planked before employees used it. The employees worked from a single 11-inch wide plank that was 47 inches to the next lower level while they washed the brick facia for the apartment building.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates and mud sills or other adequate firm foundation: On or about September 3, 2025, employees engaged in masonry activities on the fitness building of a 425 multi-family apartment complex located at 8800 South 33rd Street, Lincoln, Nebraska were exposed to fall hazards in that employees worked from a scaffold erected in mud where a least two of the scaffold poles did not bear on base plates and mudsills and at least one pole did not bear on firm foundation.
