29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) As of September 4, 2025, the employer failed to establish an effective program for the control of hazardous energy at the facility. Energy sources used at the facility include but are not limited to electric and pneumatic energy. Employees who service/complete maintenance activities on the company's Karcher Gantry Car Wash Machine (Model #CHH 8000) were potentially exposed to hazards created by unexpected energizing, startup or release of stored energy .
