29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: 1460 South Monroe St. Denver, CO 80210: On or about September 4, 2025, the employer did not ensure that foundation skylights were adequately covered. Employees finishing foundation work were exposed to slip, trip, and fall hazards of approximately 5 feet.
