(a) Noa Iniguez DBA Graydale Roofing, failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected: Citation 1, Item 1, a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of October 23, 2025. Citation 1, Item 2, a serious violation of 29 CFR 1926.503(a)(1) with an abatement date of October 23, 2025. In accordance with 29 CFR 1903.19(d)(1), abatement certification and documentation of abatement is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 4, 2025, employees conducting roofing activities (shingle installation) on an 8:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of 9-feet 4-inches at the eave, and approximately 17-feet at the gabled ridge end.

(a) On or about September 4, 2025, the portable ladder(s) being used by employees to access the roof of the structure did not extend at least three feet above the upper point of contact with the eave, nor were ladders secured in place. This exposed employees accessing and egressing the roof to a fall hazard of 9-feet, 4-inches onto concrete.
