29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which where subjected to heat, might produce flammable or toxic vapors: a) Facility - On or about August 26, 2025, an employee was exposed to explosion and struck by hazards when performing hot work on a tank by using a propane torch to heat up a square bung nut, with the tank labeled "KEROSENE" not being cleaned thoroughly to ensure there were no flammable materials present, where subjected to heat might produce flammable vapors.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Facility - On or about August 26, 2025, an employee was exposed to explosion and struck by hazards when performing hot work on a tank by using a propane torch to heat up a square bung nut, with the tank labeled "KEROSENE" not being cleaned thoroughly to ensure there were no flammable materials present, where subjected to heat might produce flammable vapors. Employees were not provided effective information and training on where Safety Data Sheets (SDSs) including, but not limited to, Kerosene and Propane were located.
