On or about August 11, 2025, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin, in the North Core Room, a lock out device was not applied to the pneumatic energy source prior to changing out the malfunctioned blow valve on the Lampe 20 core machine exposing employees to struck by hazards.

29 CFR  1910.147(e)(2)(i):  The work area shall be checked to ensure that all employees have been safely positioned or removed.  On or about August 11, 2025, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin, in the North Core Room, while testing the functionality of the blow valve on the Lampe 20 core machine, the work area was not cleared to ensure all employees had been safely positioned.  The top portion of the die box traveled in the up position when the equipment was energized exposing employee(s) to caught in between hazards from the upper die box and secure nut.
