29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   (a) Edy J Ramos Andara dba Edy J Ramos Andara failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:  Citation 1, Item 1, a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of October 24, 2025.    Citation 1, Item 2 a serious violation of 29 CFR 1926.1053(b)(1) with an abatement date of October 9, 2025.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) On or about September 4, 2025, at a jobsite located at 710 E. Arthur Dr, Oak Creek, WI, employees were conducting siding installation from a pump jack scaffold, were not protected from falling by a personal fall arrest system or guard rails. This exposed employees to a fall hazard of approximately 13 feet 3 inches to the ground below.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)   On or about September 4, 2025 at a job site located at 710 E. Arthur Dr, Oak Creek, WI, the employer had not ensured that employees repairing shingles a residential property were protected from fall hazards therefore, exposing the employees to a fall hazard of approximately 9 feet and 5 inches to the ground.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.   (a) On or about September 4, 2025, at a job site located at 710 E. Arthur Dr, Oak Creek, WI, the employer did not ensure the portable Ladder used by employees to access and egress the residential roof on the back of the home was extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately 8  feet and 11 inches to the ground below.
