29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about 8/11/2025, employees operating MTM Winder DWM 1200 machines were exposed to the unguarded rotating parts of the machine while using it to wind coils. The employer did not provide one or more methods of guarding to protect employees from the caught-in hazards. An employee was injured when she was caught-in the machine's rotating part.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. The employer failed to report a work-related in-patient hospitalization within 24 hours. On or about 8/11/2025, at approximately 10:10 pm an employee was injured in a work-related incident while operating DWM-1200 Wire Winding Machine. After the employee set the machine to spin automatically, a piece of clothing contacted the rotating component. This contact resulted in the operator being pulled into the equipment, and being hospitalized for approximately five days. The employer reported the injury on 8/26/2025 at approximately 4:18 pm, more than 24-hours after the employee was hospitalized.
