Macedonia, OH 44056, approx. 10 ft. 8 in. with a 6/12 pitch, above the ground were protected by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) On or about September 5, 2025, during a commercial construction roofing project located at 822 E. Aurora Rd. Macedonia, OH 44056, employees were exposed to a fall hazard of approximately 10 feet 8 inches while performing roofing activities on a commercial building. Employees were using a multi-position ladder to access the roof, and the employer did not ensure the ladder was secured to assist employees in mounting and dismounting the ladder.
