29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling: a) Job site 6541 Steubenville Pike Road, Pittsburgh, PA: On or about September 5, 2025 during HVAC preventative maintenance and related activities, the employer did not ensure an employee was using appropriate fall protection systems/devices where a fall hazard of up to approximately 16 feet existed.
