29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) Metal Core Department - On or about September 5, 2025, the employer did not ensure that the tongue guards on the Palmgren PowerGrind 6" Bench Grinder were properly adjusted to not exceed one fourth of an inch. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Metal Core Department - On or about September 5, 2025, the employer did not ensure that all electrical equipment was installed and used in accordance with the listing and labeling. A yellow extension cord was connected in series (daisy chained) to supply power to a power strip. The power strip was used to supply power to a blue and orange extension cord, that was used to power an orange extension cord and a portable fan.   No abatement certification or documentation is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Metal Core Department- On or about September 5, 2025, the employer did not ensure that employees were adequately protected from hazards created by rotating parts, such as the unused portions of the Amada HA-250 Horizontal Band Saw blade.    Mapes & Sprowl Steel LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1783181, citation number 1, item number 2, and was affirmed as a final order on April 8, 2025, with respect to a workplace located at 1100 East Devon Avenue, Elk Grove Village, Illinois.  No abatement certification or documentation is required for this item.
