29 CFR 1910.23(c)(8): The employer did not ensure that the top cap and the top step of a step ladder were not used as steps: On or about September 5, 2025, near the intersection of SW 248th Street and SW 135th Avenue (near U.S. 1) in Homestead, Florida, an employee was exposed to an approximate 6-foot fall hazard while working from the top of a step (A-frame) ladder when performing repairs to traffic signal devices.
