29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  A)182 Heatherdell Road, Ardsley - NY: Employees were exposed to eye/face injuries while using an electric saw and a pneumatic nail gun while conducting framing work.  The exposed employees were not protected by eye/face protection, on or about, September 3rd, 2025.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A)182 Heatherdell Road, Ardsley - NY: Employees were exposed to fall hazards of approximately 17 1/2-feet while using an extension ladder to access an upper level of a structure. The extension ladder that was used, was only extended approximately 2-feet above the landing and not 3-feet above the landing which is required by the manufacturers instructions. The employees were exposed to falls, ladder collapse hazards, on or about, September 3rd, 2025.
