29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 6 Carlyle Pl, Hartsdale - NY: An employee was exposed to fall hazards of approximately 14 1/2-feet while using an extension ladder to access a roof. The extension ladder that was used, was only extended approximately 1 1/2-feet and not the required 3 -feet above the landing, on or about, August 29th, 2025.
