29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a)Banner Associates, Inc. at 47491 240th Street, Dell Rapids, SD 57022:  On or about and at times prior to September 3, 2025, the employer did not ensure the employee was instructed to recognize, avoid, and address unsafe conditions involving wet tapping operations.  This condition exposed an employee to explosion hazards associated with conducting wet tapping activities on over pressurized Polyvinyl Chloride ("PVC") water lines.
