29 CFR  1910.252(b)(4)(iv): Where a welder must enter a confined space through a manhole or other small opening, means shall be provided for quickly removing him in case of emergency. When safety belts and lifelines are used for this purpose they shall be so attached to the welder's body that his body cannot be jammed in a small exit opening. An attendant with a preplanned rescue procedure shall be stationed outside to observe the welder at all times and be capable of putting rescue operations into effect:  During the week of September 1, 2025, at a facility located at 117315 Forward Street in Stratford, Wisconsin; A Welder/Fabricator employee entering the confined space of a tank through a 20-inch diameter manway, to grind on welded seams, was not provided for quick removal in case of emergency. The employee entering the tank was not provided with body harness and lifeline for quick removal from the confined space. The employer did not ensure an attendant with a preplanned rescue procedure was stationed outside the tank to observe the welder employee at all times.

29 CFR  1910.1026(d)(1): Each employer who has a workplace or work operation covered by this section shall determine the 8-hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI). This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section:  During the week of September 1, 2025, and after, at a facility located at 117315 Forward Street in Stratford, Wisconsin; the employer had not determined the 8-hour TWA exposure for each employee exposed to chromium (VI) during welding operations on stainless steel.

29 CFR  1910.1026(l)(2)(i): The employer shall ensure that each employee can demonstrate knowledge of at least the contents of this section and the purpose and a description of the medical surveillance program required by paragraph (k) of this section:  On September 10, 2025, at a facility located at 117315 Forward Street in Stratford, Wisconsin; the employer did not ensure that employees exposed to chromium (VI) when TIG and MIG welding on stainless steel, could demonstrate the knowledge of the contents of this section and the purpose and a description of the medical surveillance program for chromium (VI).

29 CFR  1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On September 10, 2025, at a facility located at 117315 Forward Street in Stratford, Wisconsin; the employer did not verify through written certification that a hazard assessment for the use of PPE had been conducted when employees were exposed to hazards that required equipment such as but not limited to safety glasses, hearing protection, welding helmets and clothing, and gloves.
