29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) ACC Foods LLC - Bun room area - Bun making machine:  An employee operating the YJ-241 PRO AUTO DOUGH KNEADING/SHEETING MACHINE (Line #2)  was exposed to multiple crushing hazards when he used his fingers to push the dough towards the pinch points between the rollers that pulled the dough into the machine, occurred on or about 12/16/2025.  b) ACC Foods LLC - Bun room area - Conveyor system :  An employee operating the conveyor system attached to the YJ-241 PRO AUTO DOUGH KNEADING/SHEETING MACHINE (Line#2) was exposed to multiple crushing hazards when he used his fingers to push the dough towards the pinch points between the rollers that pressed and pulled the dough into the machine, occurred on or about 12/16/2025.  c) ACC Foods LLC - Bun room area - roll-up roller wheel :  An employee operating the roll-up roller wheel system attached to the conveyor line attached to the YJ-241 PRO AUTO DOUGH KNEADING/SHEETING MACHINE (Line#2) was exposed to multiple crushing hazards when he used his fingers to push the dough towards the pinch points between the roller and conveyor belt that created the bun end product, occurred on or about 12/16/2025.  d) ACC Foods LLC - Dumpling room area - Line #2:  Packing employees were in close proximity to the unguarded pistons and dough cutter on the XLV dumpling forming press line #2, resulting in employees exposure to crushing hazards, occurred on or about 12/16/2025.  e)  ACC Foods LLC -  Noodle line #1 :  The noodle line #1 next to the control panel number XL21 had unguarded conveyor.  Employees ensuring that dough is placed straight in the conveyor belt are exposed to the unprotected ingoing nip points, which exposes them to crushing hazards, occurred 12/16/2025.  f)  ACC Foods LLC -  Noodle line #2 :  The noodle line #2 after the dumping station (after the dough is mixed) had an unguarded inclined conveyor belt.  Employees ensuring that dough is placed straight in the conveyor belt are exposed to the unprotected ingoing nip points, which exposes them to crushing hazards, occurred 12/16/2025.  g)  ACC Foods LLC -  Noodle line #2 :  The noodle line #2 after the inclined conveyor belt had an unguarded conveyor.
