Rosemont Lane, Oak Creek, WI 53154, the employer did not ensure the employees assisting in roofing operations (ground worker) were protected from potential falling objects from above by the use of a protective helmet. This exposed the employees to a struck-by hazard from a height of up to approximately 18 feet 9 inches.

(a) On or about September 9, 2025, at 10523 S. Rosemont Lane, Oak Creek, WI 53154, an employee conducting roofing activities on a 10:12 pitch roof was not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.
