29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about September 9, 2025, at 10655 S. Sangri-la Court, Oak Creek, WI 53154, employees installing a roof on a residential home roof with a 11:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 15 feet 5 inches from the roof eve to the ground below on the front of the home. 5 Star Innovation Co LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1813314, citation number 1, item number 3a and was affirmed as a final order on May 8, 2025, with respect to a workplace located at 308 Saddle Ln, Fox River Grove, IL 60021.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) On or about September 9, 2025, at 10655 S. Sangri-la Court, Oak Creek, WI 53154, employees conducting roofing activities on a 11:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries. 5 Star Innovation Co LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(17), which was contained in OSHA inspection number 1813314, citation number 1, item number 3c and was affirmed as a final order on May 8, 2025, with respect to a workplace located at 308 Saddle Ln, Fox River Grove, IL 60021.
