Sangri-la Court, Oak Creek, WI 53154, employees conducting roofing activities on a 11:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.  5 Star Innovation Co LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(17), which was contained in OSHA inspection number 1813314, citation number 1, item number 3c and was affirmed as a final order on May 8, 2025, with respect to a workplace located at 308 Saddle Ln, Fox River Grove, IL 60021.
