29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  (a) Shop Area - On or about August 18th, 2025, an authorized employee did not receive training on the methods and means necessary for energy isolation. Specifically, the steps to take when lockout or tagout devices must be temporarily removed from an energy-isolating device, allowing the machine or equipment to be energized for testing or positioning. As a result, the employee was exposed to the ingoing nip points of the chain and sprocket connecting the motor to the auger while testing the motor of a Truck Craft TC-140 D-Icer (tailgate salt-spreader).  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

1910.147(f)(1)(ii) Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed:  (i) Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section stating "(e)(1) The work area shall be inspected to ensure that nonessential items have been removed and to ensure that machine or equipment components are operationally intact"; and, (ii) Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section stating "(e)(2)(i) The work area shall be checked to ensure that all employees have been safely positioned or removed"  (a) Shop area - On or around August 18, 2025, an employee was testing the motor of a Truck Craft TC-140 D-Icer (tailgate salt-spreader). During this testing, the employer did not ensure that all employees were removed from the work area, which resulted in an employee being exposed to the ingoing nip points of the chain and sprocket that connects the motor to the auger.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or around August 18, 2025, an employee experienced a partial amputation of the middle finger on their right hand. This amputation was not reported within the 24-hour timeframe and was instead reported on August 26, 2025.  No abatement documentation or certification is required for this item.
