29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer:  On or about September 10, 2025, employees were exposed to a fall hazard of 4-20 feet while being lifted inside of a man basket attached to a Hyster forklift (Model H50XM and S/N H177B45245A). Employees were trained incorrectly to use a PFAS harnesses with lanyard inside the man basket incorrectly in that they were using a support piece of the guard rail system as an anchorage point as part of the personal fall protection system.

Employees were using a man basket lifted by the Hyster forklift (Model H50XM and S/N H177B45245A) and PFAS harnesses with 6ft lanyard while utilizing a support piece of the guard rail system as an anchor point.

Employees are tasked with service and maintenance of equipment including but not limited to dust collectors, aggregate conveyor belts, and shop equipment.
