29 CFR 1910.212(a)(3)(iii):Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. a) On or about September 3, 2025, the employer failed to ensure that special hand tools were used in a manner that prevented employees from placing their hands in the danger zone during shaft changeover operations. Specifically, an employee was manually decompressing a bladder on a flex machine shaft using their fingers, while another employee pulled the shaft from the opposite end, resulting in a pinch point between the shaft collar and the cardboard core. As a result, employees were exposed to caught-in hazards that resulted in amputation injuries.

29 CFR 1910.219(c)(2)(i):All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. a)On or about September 3, 2025, the employer failed to guard exposed portions of horizontal shafting located seven feet or less from the floor. Specifically, the CSHO observed unused portions of horizontal shafting on the flex machine that were not enclosed by a stationary casing or a trough, leaving rotating components accessible to employees working in the area. As a result, employees were exposed to caught-in hazards from unguarded rotating shafting.

Additionally, the shaft ends contained unused keyways that were not filled or covered, creating additional projection hazards. As a result, employees were exposed to struck-by and pinch point hazards.
