29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv): a) Press Brake Room - On December 19, 2025, the employer did not ensure that the flywheel located on the M85 Walsh Model 24 Press and Die Co. mechanical power press was fully enclosed by a guard. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Assembly Department - On September 11, 2025, the employer did not ensure that one or more methods of machine guarding was used during the operation of the Edward Segal riveting machines #1 and #11. No abatement certification or documentation is required for this item. MAC Manufacturing dba Saturn Manufacturing dba Aeronautical Electric was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1795918 citation number 1 item number 2 and was affirmed as a final order on March 18, 2025, with respect to a workplace located at 5656 N. Northwest Highway in Chicago, IL 60646.
