The employer did not perform an adequate and complete hazard assessment for job and tasks, such as but not limited to bin entry, leg cleaning, and stacker operating, plant operator, truck driver, and others.

29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  At the worksite located at 28700 South State Route T Garden City, MO: Employees were exposed to respiratory hazards. Employees were required to wear half face respirators in the warehouse and at times outside and by maintenance. The employer did not establish and implement a written respiratory protection program with worksite specifics procedures, fit testing, and medical evaluations of employees, nor training on proper storage and maintenance of respirators.

The employer did not establish a written hazard communication program that included information such as a chemical inventory list, training to be provided to employees, and any labeling requirements. Employees use and have a chance of exposure to chemicals such as but not limited to carbon monoxide, 409, propane, zinc sulfate monohydrate, and others.
